I accidentally created a new profile for a client that already had one. What do I do now?

If the clients are in HMIS, email HMISHelpdesk@ihcda.in.gov.

If the clients are in DV, email DVHelpdesk@ihcda.in.gov.

In your email, include the following:

  1. Subject Line that Contains "Client Merge"
  2. Target Client ID (client profile you want to keep)
  3. Source Client ID (client profile that will be merged and deleted)

What not to include:

  1. Client Name
  2. Client SSN
  3. Client DOB
  4. Any Other Identifying Client Information (PII)
